Is your child getting the diabetes care they need at school?
When you share your story, you become a FOLLOWT1Ds parent—a community of T1D parents advocating to ensure all students with T1D nationwide have access to the accommodations they need at school.
This form will allow you to share any issues related to your child’s diabetes management at school. Your information will help us:
Advocate for Change: Use your input to identify systemic issues and push for better school policies and accommodations for students with T1D at local and national levels.
Support our Community: Your information will help us to know how to best support you at this time in addition to providing helpful resources and to connect you with other T1D families in your area to support collaboration and advocacy if requested.
Raise Awareness: We value your privacy. General, non-identifying information you share may be used to increase awareness and strengthen our advocacy efforts.
Improve Policies: Your input highlights common challenges, guiding our efforts to shape more effective policies and solutions for students with T1D.
Participation is entirely voluntary, and all information is kept confidential.
